First of all, we were able to successfully do this once, but now cannot reproduce this and I'm completely baffled.

What we are looking to do is take a 10-question quiz and put three questions at the end of Module 1, Module 2 and Module3 (4 questions). 

I've matched all the settings to the course that we produced prior but in this new course, when we played it in our LMS during testing, the users found that only Q8, Q9 and Q10 were graded, as if there were no Q1-7 at all, yet, still the total point value =100.   In this case, the tester got Q9 wrong and Q8 & Q10 correct and the summary showed he only achieved 20%, so he failed based upon needed our standard 80% to pass.

 

The Quiz ID for all slides is the same, but the interaction ID is different per quiz slide.  We have it set so that they can answer the question twice before failing the question.  The quiz can be taken infinite times.

This may not be the best avenue and I'm seeking help from the community.  Let me know if you might be able to help me troubleshoot.
